We need to hold personal information about you on our computer system and in paper records to help us to look after your health needs. All information we hold for patients on the computer or in their medical records is highly confidential and would not be released to any other third party without the written consent of the patient. Information is only passed on with your consent, within the confines of the NHS, by law or if in the case of public health interest.
To ensure your privacy, we will not disclose information over the telephone unless we are sure we are talking to you. We will not leave messages with family. Information will not be disclosed to family, friends, spouse or carers unless we have your written consent. If you would like your family, friends, spouse, Carer etc to deal with any concerns for you and have access to your medical record, you will need to complete and sign a consent form.
